Sharing Your Wishes
One of the hardest things for most of us to talk about is what we’d like done for us as we approach the end of our lives. It’s possible that we may reach a point where we are unable to speak for ourselves. That’s why it’s important for all of us to let our loved ones know what we would wish for ourselves as the end of life draws near.
Share your wishes now so that your choices can be honored. Once you have advanced care planning in place along with a health care proxyyou’ll have a greater feeling of ease about the future. Talk to your designated health care agent soon and let that person know what you’d like to have done. Don’t leave these decisions up to others.

Call or email today to request a free “Sharing Your Wishes” packet. This packet includes a workbook to help you through the steps of how to begin the conversation on making your advanced care wishes known, and also a health care proxy form to be filled out. Give yourself and your loved ones peace of mind. Call the Office for the Aging at 253-1226 today to request your packet, or click on the Request for Assistance button found below.
Store Your Wishes

Now that you’ve shared your wishes, you may want to store your completed health care proxy form at a secure on-line website where it can be retrieved by you, your health care agent, doctor, clergy or family member. To find out more about this very safe and free method of storing your important information call the Office for the Aging at 253-1226 for an application or go online to www.AssuringYourWishes.org.
