Long Term Care Office

Long term care is a reality that many of us may face someday- either personally, or for one of our family members. Finding the right service can be difficult and confusing.

At the Cayuga County Long Term Care Office, our goal is to assist people with chronic care needs maintain their independence. Members of our staff want to provide you with the information that will help you make informed choices about appropriate, cost-effective services. Long Term Care can be very confusing and we are here to help you or your loved one receive the best care possible.

Services include information and referral, along with intake and screening.

Our nurses will provide a free complete in-home assessment or evaluation of your needs, care plan development, case management, community PRI assessment (screening for nursing home placement), and access to Medicaid homecare and Office for Aging programs such as caregiver support, respite and home care (EISEP).

The Cayuga County Long Term Care Office, with cooperative efforts between the Cayuga County Department of Health and Human Services and the Cayuga County Office For the Aging, is your one stop place to help you find cost-effective, high quality long-term care services for yourself or someone you know, regardless of age or income.

If you think you have an immediate need for Personal Care Services (PCS) or Consumer Directed Personal Assistance Services (CDPAS), such as housekeeping, meal preparation, bathing, or toileting, your eligibility for these services may be processed more quickly if you meet the following conditions:

·         You have no informal caregivers available, able and willing to provide or continue to provide care;

·         You are not receiving needed help from a home care services agency;

·         You have no adaptive or specialized equipment or supplies in use to meet your needs; and

·         You have no third party insurance or Medicare benefits available to pay for needed help.

If you don’t already have Medicaid coverage , and you meet the above conditions, you may ask to have your Medicaid application processed more quickly by sending in: a completed Access NY Health Insurance Application, the Access NY Supplement.  If needed; a physician’s order for services; and a signed *“Attestation of Immediate Need.”

If you already have Medicaid coverage that does not include coverage for community
